Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 12/2/2025, and subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 5 of 5 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. The child stepping stones must be placed in an area with resilient surfacing. 2. The soiled diaper container must be lined.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 12/6/2024, and are subject to changes pending programmatic review. Compliance Officer #1: Chloe-James Rossi The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but is not limited to: *Ensure fire drills are conducted at different times of the day, and *Please submit a Space Change Application to include diapering in the Preschool room. There were 4 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: There were no deficiencies observed at the time of the Annual Compliance Inspection conducted on December 5, 2023, Compliance Officer #1: Stacy Marchelli Compliance Officer #2: Dawn Rathburn A complete inspection was conducted at this time. Two fingerprint clearance card were verified through the DPS website during the inspection. During the exit interview, the following items were discussed but are not limited to: *Ensure medical symptoms are included on medical permission form.
