| Initial Comments: No deficiencies were found at the time of the Compliance Inspection conducted on 9/4/2025, and are subject to changes pending programmatic review. The following was discussed, but is not limited to: *Ensure the misters in the outdoor activity area do not obstruct the view of the children, *Ensure bathroom storage is in a closed container, and *Ensure the address on staff criminal history affidavits reflects the address of the facility. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. | |||

| Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 9/17/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 2 of 2 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Ensure drinking water is accessible to enrolled children. 2. Ensure rosters include children's first and last names. 3. Ensure each child's toothbrush is accessible to each specific child only, or keep them inaccessible. 4. Ensure children's Record cards are complete with all required information. 5. Ensure fire drills are conducted at different times of the day, 6. Ensure stickers are removed from diaper surfaces. ***A reminder of the license expiration date was stated to the Director. | Initial Comments: The purpose of the inspection was to conduct complaint case #61709 investigation on September 18, 2023. The investigation was completed on September 19, 2023. A compliance inspection was also conducted. Compliance Officer: Stacy Marchelli Ratios observed were: Early Head Start: 2:6 Head Start: 2:16 There was one (1) staff file reviewed during this investigation. Documentation observed was: 1. Glendale Police Report 2. Staff statements regarding the incident 3. Child Incident Report Upon completion of the complaint investigation, it was determined from observation, interview, and documentation the allegation was substantiated.
