A safe indoor and outdoor environment was not provided for the children. The outdoor environment had a trash bag on the playground and three (3) used plastic cups. The red shelter sand table structure needs to be power washed as well as the cover to the sand table and the child size picnic table. The fake brick is chipping and flacking off at the bottom right of the building, as you face the building from the playground area, exposing the material underneath.

Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented lockdown drill was in May of 2022. The emergency drill prior to that was 1/27/22. There was no documentation of an emergency drill for April 2022, which was at least three months from the January emergency drill.
