| The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Today I observed trash on the playground area, weeds and vines growing through the fence on the playground area, the large equipment needs power washing including the roof tops, the tops of the tunnel and the covered slide, as well as the two (2) playhouses. Ground cover is exposed at the yellow climber posing a tripping hazard for children and staff. | |||

At the fence gate on the lower back left corner I observed there was a 4 inch opening between the fence pole and the fence gate. I observed there was an opening at the right end of the fence between the fence and the black ground barrier that measures 4.5 inches and the opening on the right end and measures 6.5 inches on the left end and there is a drop off from the ground between the fence and the ground. | |||

The sandbox was not in use on the last RU visit in October. Today the sandbox was open for children but no repairs have been made on the sandbox roof. | |||
