2026-07-01 Incident Report Supervision Concern Substantiated
Findings: Supervision Concerns. There were concerns that a child had been unsupervised for an extended period of time. The health department confirmed that a child had escaped the classroom out to the playground and was unsupervised for approximately 23 minutes. Additionally, the same staff member had previously fallen asleep while supervising children during nap time. Resolution: This provider has required the staff member to take active supervision trainings, rearrange the cots during nap so that all children are visible, and take walks if they feel drowsy while supervising.
