| Findings: Supervision Concerns. There were concerns regarding background checks, supervision/ratios, and behavior management/discipline practices. During the investigation, the provider admitted to the inspector that a staff member was working at the facility without receiving an enhanced background check clearance or temporary City of Coeur d'Alene provider license. At the time of investigation, all children were properly supervised and the ratios were in compliance. When asked to view footage of possible mistreatment of children, the provider claimed the system was not currently working. However, a former employee had been terminated due to actions toward a child on the playground. Resolution: The staff member without proper clearances was fired due to inappropriate discipline actions. The health department returned to try and view the video footage but the system was still down and unable to be retrieved. | |||

| Findings: Child Abuse and Neglect. There were concerns of a staff member using excessive physical force with a child. At the time of investigation, it was confirmed that the staff member did use excessive force when handling a child. Resolution: The facility self-reported the incident and the staff member was immediately terminated from employment. Law enforcement and the parents of the child were alerted. The staff member was issued a citation for injury to child. | |||
