Noncompliance Area: While in the 3 Y/o room a child was observed to have their face covered by a blanket which can be dangerous to the child and could not allow staff person #1 to fully supervise the child. While in the Infant room the room was observed to be so dark that it was difficult to see the children, making it difficult to ensure full supervision by staff person #2. While in the 2 y/o room Staff person #3 was standing behind the changing table obstructing their view of where a child was observed to be sleeping, making it so that not all children could be adequately supervised.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

A facility person's record shall include the criminal history record, the FBI clearance, child abuse registry clearance information and a copy of the disclosure statement as required under the CPSL.

Noncompliance Area: The outdoor equipment which has three embedded anchors was observed not to be mounted above a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: The file for staff person #3 was observed not to contain documentation of a minimum of 6 clock hours of child care training hours within the last 12 months.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.

Noncompliance Area: The file for staff person #1 was observed not to contain documentation of a valid Pennsylvania State Police Clearance. This is evidence by only the request for clearance being on file. Also, the file for staff person #2 whose date of hire was 3/28/2018 was observed not to contain documentation of request for a Pennsylvania State Police Clearance or Child Abuse clearance prior to the start of employment. This is evidence by the Pennsylvania State Police clearance being applied for until 4/2/2018 and the Child Abuse Clearance being applied for until 5/29/2018.

Noncompliance Area: One protective receptacle cover was observed to be missing from an electrical outlet next to the refrigerator in the preschool 3 classroom.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Damaged plaster was observed behind the trash can and stacked chairs in the older toddler classroom.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: On 1/19/2018, child #1 was observed to be napping in an infant rocker. This rocker does not allow the child to be placed to rest flat on his back as recommended by the American Academy of Pediatrics.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: RECITE: On 1/19/2018 the hot water temperature in each sink of the child care facility where children was their hands was observed to be greater than 110 degrees. This is evidenced by the temperature being 117.4 degrees in the children's restroom, 117.7 degrees in the infant room, 119 degrees in the Pre-K 3 room, 119.5 in the Twos room, 120 degrees in the Ones room, and 120 degrees in the Pre K room. Hot water temperatures was previously cited at the facility's renewal inspection on 12/21/2017

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: The emergency contact form on file for child #4 was observed not to contain a telephone number for one of the emergency contact persons.

Correction Required: Emergency contact information is required for each enrolled child. Emergency contact information must reference who shall be contacted in an emergency.

Noncompliance Area: The emergency contact form on file for children #1 and #3 were observed not to contain complete work information for the enrolling parent.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.

Noncompliance Area: The emergency contact form on file for child #2 was observed not to contain complete health insurance coverage information.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The emergency contact form on file for children #3 and #4 were observed not to contain complete contact information for the person's whom the children are authorized to be released to.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The health report on file for children #1 and #3 were observed not to contain a health report dated within the child's first 60 days in care. This is evidenced by child #1's date of admission being 9/11/2017 and a health report not being obtained until 11/28/2017 and child #2's date of admission being 9/11/2017 and a health report not being obtained until 11/30/2017. Also, the file for child #5 was observed not to contain a complete health report only a shot record was on file for child #5.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: The diaper genie being used to dispose of soiled diapers in the infant room was observed not to be hands free. This is evidenced by a button needing to be pushed to open the lid of the diaper genie.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: The bottles being used in the infant room were observed not to be labeled for the child whom they were intended for.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: The written notification of safe routes posted were observed not to be complete.

Correction Required: Written notification of safe routes shall be posted by the operator at a conspicuous location in the child care facility.

Noncompliance Area: One pack and play in the infant room was observed to have a full sized crib sheet placed on the sleeping surface. This creates an excessive amount of fabric on the sleeping surface creating a suffocation hazard to the children sleeping in the pack and play. Another pack and play where an infant aged child was observed napping was observed to have an extra blanket placed on the surface not being used to cover the child. The extra blanket also, created an excessive amount of fabric on the surface creating a suffocation hazard to the children in care. In the Pre K room the cord for the refrigerator was observed to be accessible and hanging loose in a manner that allows for the cord to be looped creating a choking hazard to the children in care.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Driveway salt was observed being stored in an accessible area to the children in care in the front lobby of the child care facility.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Shaving cream was observed being stored with the arts and crafts supplies in the terrific 2's room. The operator confirmed that the shaving cream was being used for arts and crafts. The shaving cream was labeled keep out of reach of children.

Correction Required: Arts and crafts materials shall be nontoxic.

Noncompliance Area: The hot water temperature in each room was observed to exceed 110 degrees. In the infant room the temperature was 140 degrees, Pre K room 138.9 degrees, in the Terrific 2's room 134.9, Ones room 138.3 and in the preschool room 126.1.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: A hand washing sign was observed not to be placed above the diaper changing table in the ones classroom.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: The facility was observed not to have been completing a fire drill at least every 60 days.

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.

Noncompliance Area: It was observed in room 5 that there was not a written plan of daily activities and routines in place in the classroom, nor was there a written plan of daily activities posted in this classroom.

Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group. The written plan of daily activities shall be posted in the group space.

Noncompliance Area: The file for staff person's #1 and #2 were observed not to contain 2 written non family references.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person

Noncompliance Area: On the date of inspection the cables and power cords for each cable box located in each child care space except for those located in room 1, were observed to be loose and dangling on the floor in a manner that can cause the cables to be looped creating a strangulation hazard to the children in care and poses a threat to the health and safety of the children in care.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The emergency plan on file at the child care facility was observed not to specify an off site shelter nor how the children would be transported to the emergency shelter.

Correction Required: The facility shall have an emergency plan that provides for evacuation of children from the facility building and evacuation of children to a location away from the facility premises. The evacuation routes and evacuation plans to exit the building may be the same as those required by 3270.94 (f) and (g) (relating to fire drills).

Noncompliance Area: The file for staff person's #1 - #3 were observed not to contain documentation of training in the facility's emergency plan at the time of initial employment.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: On the date of inspection the disclosure statement on file for staff person #1 was observed not to be dated, the Pennsylvania State Police Criminal History and Pennsylvania Child Abuse clearances on file for staff person #2 were observed to be for volunteer purposes making them not valid, and the disclosure statement on file for staff person #4 was observed not to be dated or contain a witness signature.

Noncompliance Area: In the infant room a bumbo seat was observed without the safety straps.

Correction Required: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement.

Noncompliance Area: The file for staff person #1 was observed not to contain documentation of a health assessment or TB being completed within the last 12 months.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: It was observed that there was not file present for staff person #2.

Correction Required: An individual record is required for each facility person.

Noncompliance Area: It was observed that the facility did not send out a copy of the emergency plan to the county emergency management agency.

Correction Required: The operator shall send a copy of the emergency plan and subsequent plan updates to the county emergency management agency.

Noncompliance Area: The file for staff person #1 the facility director was observed not to contain documentation of one year experience working with children.

Correction Required: A director shall have attained a bachelor`s degree from an accredited college or university in early childhood education, child development, special education, elementary education or the human services field and 1 year of experience with children.
