|Findings: Observed the center having an ant infestation. Please send me a copy of the receipt once this has been corrected.|
|Findings: LS conducted a complaint investigation this date. The complaint alleges that her 2 year old told her that teacher slapped him. LS discussed with the Director the regarding the allegations. The child's father came informed Director that his son told him that teacher slapped him in the face. The parent came to the day care the next day to talk to teacher, but she didn't come to work that day. The next day the parent spoke to teacher who denied ever hitting his son. LS spoke with teacher regarding the complaint. The teacher states that on that particular day she saw that the child in question eating someone else's lunch when she grabbed his face and told him to spit out the food and do not eat someone else's lunch. She states she didn't hit the child, but she grabbed his face. She states they do not hit the children, she put them in time out as a form of discipline. LS received written statements from director and teacher.|
|Findings: Director states she hired a new staff person who only comes in the morning to drive the children to school. No evidence of a current Notarized Release of Information and Fingerprints for Ms. Holt. Ms. Holt works at another center during the daytime.|
|Findings: Observed the boys bathroom upstairs having urine stains on the toilet seat. Observed in the girls bathroom downstairs and boys and girls bathroom upstairs missing paper towel accessible to the childre|
|Findings: Observed 1 school age teacher missing 32 hours of continued training, this is training that was due 2016, 2017 and now 2018. Also still missing the required training ADA and Basic Health & Safety. LS will discuss with OCC Supervisor regarding this situation.|
|Findings: Observed the aide missing 10 hours of continued training from 2017 & 2018.|
|Findings: All applicable admission and documentation requirements were not met for each child.|
|Findings: The center did not have a toilet and sink for every 15 children who are 2 or older.|
|Findings: The activity area contained a potential hazard to child health or safety.|
|Findings: Each new hire did not receive a complete employee orientation as required.|
