|Findings: LS conducted complaint investigation this date. Complaint alleges that there was an injury in the infant room and the parent did not receive an incident report. LS observed classroom 4 and 8 having Aides supervising the classroom.|

|Findings: LS conducted complaint investigation on today's date. Complaint alleges that the Caller reported that she received a phone call that her child was bit on the shoulder by another child. Caller states that this child has repeatedly bit or hit her child and the Director has been unresponsive. LS observed that the center does not have the required incident report on file regarding the incident of a child that was bitten in care.|
|Findings: LS conducted complaint on today's date. Complaint alleges that the Caller reported that she received a phone call from the Director that her child was bit on the shoulder by another child. Caller states that this child has repeatedly bit or hit her child and the Director has been unresponsive. Director stated she is unable to do anything about the child causing the incidents because he has special needs and it is against the ADA to put a disabled child out or refuse service. The Director stated she would assess the situation to determine what to do. Caller also alleges that there was another incident where her child came home with missing hair from her head and the child's hair has not grown back. Center alleged that child had ringworm. According to caller, child was taken to the doctor who stated that it the hair lost was due to trauma. Caller also stated that child had sustained a burn. Both incidents allegedly occurred prior to October 2017. The teacher informed LS that at times she is left alone with the child in question along with 9 other children because the center is short staffed due to staff calling out.|
|Findings: LS observed 1 bathroom stall inoperable in the 2 and 3 year old area.|
|Findings: Observed 1 new staff person working with the children without OCC receiving the required paperwork. Missing the Notarized Release of Information, Fingerprints and medical report for Ms. Michelle Coates.|
|Findings: Observed a torn diapering pad in room 2 (Infant room).|
|Findings: Observed in classroom 8 the teacher's purse accessible to the children.|
|Findings: All applicable admission and documentation requirements were not met for each child.|
|Findings: The posted staffing pattern did not meet all requirements.|
