Noncompliance Area: The side doors on the vanity in the preschool bathroom were not locked allowing the Comet and Clorox Toilet Wands, labeled "keep out of the reach of the children," to be accessible to children in care.

Noncompliance Area: The orange tunnel on the preschool climber in the front play space is cracked. The damaged plastic created a sharp edge and a pinch hazard for children.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Description: Building Surfaces

Noncompliance Area: A piece of the baseboard in the toddler area was pulling away from the wall. The end post of the fence adjacent to the garage was not secured in the ground and easily shifts. This creates a significant gap that is large enough for a child to pass through.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Handwashing signs were not posted at the changing table in the infant classroom and the changing table and sink in the toddler classroom.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: The file of facility person #1 contained documentation of emergency plan training dated 6/4/2018 and the file of facility person #2 contained documentation training on 6/5/2018. Facility person #1 and 2 did not contain documentation of emergency plan training the year prior. Facility person #1 and 2 now have current emergency plan training.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: The file of facility person #1 and 2 did not contain documentation of 6 hours of annual child care training.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.

Noncompliance Area: The upstairs child care space contained a surge protector which had 2 electrical outlets that did not contain protective electrical covers.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: The play space in the back of the facility contained 2 railroad ties, which make up a part of a wall, were coming apart at the touch.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: The infant room contained a portable space heater on a shelf.

Correction Required: Portable space heaters are not permitted.

Noncompliance Area: In the preschool/SA classroom, a Lego platform was broken and cracked. In the play space outside the infant room, a play grill had bar with cracked and broken plastic on the top. In the front outdoor play space, a slide had a broken or missing bolt which attached support bar from the slide to the climber.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the toddler space, Disney foam puzzle pieces contained numerous teeth marks.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: When asked how infants are placed in their cribs when they nap, the facility person stated they placed the one infant on their side since they are a side sleeper.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: The file of facility person #1, with a start date of 2/7/2008, contains documentation of annual emergency plan training dated 6/6/2017, but does not contain any documentation of the prior training.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: In the play area outside the infant classroom, a nail and piece of wood were protruding off of the side of the building. In the front play space, the roof of the shelter is peeling.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: The outside door off the toddler classroom contains peeling paint.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: In the upstairs restroom, each side of the toilet has an exposed bolt.

Correction Required: Toilet areas and fixtures shall be cleaned daily and be in good repair.

Noncompliance Area: The outdoor preschool/school-age playspace contained a tee ball tee which was cracked, a playhouse with a crack on the door hinge and a playhouse with cracks by the outside window all exposing pinch and crush points for the children's fingers. The handle of the three wheel bike was broken and hanging off the bike.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: At approximately 9:57, child #1 was asleep in a carseat inside a crib. Staff #2 stated it was their child and would only be in there briefly so the children could not grab him through the slats of the crib.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: While observing the nursery, child #2 and 3 were placed in highchairs and given their lunch without having their hands washed.

Correction Required: A staff person shall ensure that a child`s hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: The file of staff #2, with a start date of 7/6/2017, contained a health report dated 7/12/2017, which is after the their starting date with children.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The file of staff #1 contained a disclosure form signed by the staff person but lacked a witness signature and date which the form was signed leaving it incomplete. The file of staff #2 did not contain documentation of a completed State Police Clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person`s record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #2 may not work in a child care position at the facility.

Noncompliance Area: The file of staff #2, identifed as the facility director, did not contain documentation of education.

Correction Required: A director shall have attained one of the following qualification levels specified at 3270.34(b)(1) - (4). A facility person`s record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: At approximately 9:55am, I arrived at the facility and staff #2 let me in the door, I was guided through the toddler classroom to the nursery where there were 3 children 4 months to 1 year of age. When asked who was supervising the children when she answered the door, staff #2 stated staff #3, who was responsible for 4 children 2-3 years of age, was standing at the door. This left staff #3 out of ratio supervising 7 children between 4 months and 3 years of age.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).

Noncompliance Area: The wooden fence in the toddler play space had a broken slat leaving sharp edges exposed and accessible to the children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility`s outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: In the front eating area of the toddler room, there were many foam puzzle pieces with teeth marks and small pieces coming apart that could be placed in a child's mouth.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: A daily activity schedule was not posted in the infant room.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: The file for staff person #1 lacked current emergency plan training updated within the last year.

Noncompliance Area: In the toddler outdoor play space, there was splintering in one area on top of the fence by the facility and gate, and there was splintering in a middle section of the back wall and a section in the right corner. In the outdoor preschool/ school-age play area, there was a section of the inside of the roof of the shelter that had splintering in a few areas on the particle board.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility`s outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
