Noncompliance Area: The file of facility person #1 contained a health report dated 8/3/2016 with an update on 9/6/2018, exceeding the requirement of having an updated health report withing 24 months. Facility person #1 now has a current health assessment.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The file of facility person #1 contained documentation of emergency plan training dated 6/4/2018 and the file of facility person #2 contained documentation training on 6/5/2018. Facility person #1 and 2 did not contain documentation of emergency plan training the year prior. Facility person #1 and 2 now have current emergency plan training.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: The file of facility person #1 and 2 did not contain documentation of 6 hours of annual child care training.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.

Noncompliance Area: The upstairs child care space contained a surge protector which had 2 electrical outlets that did not contain protective electrical covers.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: The play space in the back of the facility contained 2 railroad ties, which make up a part of a wall, were coming apart at the touch.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: The infant room contained a portable space heater on a shelf.

Correction Required: Portable space heaters are not permitted.

Noncompliance Area: In the preschool/SA classroom, a Lego platform was broken and cracked. In the play space outside the infant room, a play grill had bar with cracked and broken plastic on the top. In the front outdoor play space, a slide had a broken or missing bolt which attached support bar from the slide to the climber.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the toddler space, Disney foam puzzle pieces contained numerous teeth marks.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: When asked how infants are placed in their cribs when they nap, the facility person stated they placed the one infant on their side since they are a side sleeper.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.
