Operator was not the person on-site, providing care to children. Both additional caregivers were present providing care to children during today’s visit and stated that Mrs. Mullings, Owner/Operator was not present.

Individuals who provide care for 5 hours or more a week, during planned absences of the operator, had not completed first aid and CPR as described in Rule .1702(b)(2). An additional caregiver who was observed caring for children during today’s visit did not have First Aid and CPR training through an approved agency.

All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. In the indoor environment small areas of chipping paint were located on the walls in the childcare room and on the door leading to the bathroom used by children. Chipping paint was also observed exposed on the top, right side of the infant crib. On the outdoor play area, one broken swing had been flipped over the top of the play set, but still remains accessible to the older preschool and school age children enrolled.

Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. A large black and brown dog located on the premises did not have record of up-to-date pet vaccinations for review.

Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. The operator’s ITS-SIDS training expired on October 6, 2023.

Prior to enrollment of children in a Family Child Care Home, and before animals come into the Family Child Care Home, the operator did not obtain each parent's signature on a form acknowledging the type of animal located on the premises and where the animal will be kept during child care operating hours and /or the signed form was not maintained in the child's file. Six children's files did not have a acknowledgment form of an animal being on the premises.

A valid qualification letter was not on file and available for review at the facility. The household member and additional caregiver did not have verification of a Criminal Background Qualification letter on file for review. I verified in the criminal records system that she was qualified to work with children on September 18, 2020.

Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. A bottle of baby oil and a container of Colgate Max Fresh toothpaste was stored on top of the sink, in the bathroom used by children. A bottle of hand sanitizer and Degree Men’s Antiperspirant was stored in an unlocked cabinet, under the sink, in the bathroom used by children.

Operator was not the person on-site, providing care to children. Two additional caregivers were present providing care to children. The operator of the family child care home arrived at the facility at 1:15pm and stated that she is working outside of the home during first shift.

In the indoor environment I observed chipping paint on the walls and chipping paint on an infant crib. There was a broken piece of sheetrock near the entrance of the child care room. I observed dirt on the walls, cabinets, and paper towel dispensers. On the outdoor play area I observed big tree limbs, a broken swing, and pieces of torn plastic sticking up and exposed from under the mulch surfacing.

Items used for starting fires, such as matches, accelerants, and lighters, were not kept in locked storage. Lighter fluid and charcoal was observed stored under a grill in the back yard accessible to children.
2024-01-30 Violation 706 .1719 (a)(7)
Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. In an unlocked cabinet in the bathroom was an aerosol can of antiperspirant and an aerosol can of hair spray. In the child care room, on an open shelf, was an aerosol can of Lysol spray.
