In the children's bathroom, I observed VO5 shampoo and conditioner in the shower. Bulk soap and hydrogen peroxide was stored on the sink. You removed these items from the children's bathroom during the visit.

Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. In the childcare room 2, a container with plastic bags was sitting on a table. Diapers were stored on the changing table in the original plastic packaging. You placed these item above 5 feet during the visit.

A electric screw driver was left out accessible to children. You placed the screw driver inaccessible to children during today's visit.

Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. Pet vaccinations expired on 5/17/19.
