Findings: Per allegation and based on the Surveyors' observations of video documentation and staff interview, it was determined that the licensee placed enrolled children's heath, safety, and welfare at risk of harm when the licensee allowed staff #1 to care for the enrolled children in the 1's classroom. The Surveyors observed the following via live video documentation on 09/23/21: * From 9:09 a.m. to 9:53 a.m., staff #1 sat on the floor in the carpeted area while the children sat or walked around the room. Staff #1 did not interact with the children during this time and appeared to be falling asleep at one point. *As staff #1 was standing in the carpeted area of the room, she roughly grabbed a child by one arm, and swung the child upward and then sat them roughly on the floor. *At 10:50 a.m., staff #1 started to prepare lunch for the children. Staff #1 gathered the lunch plates and placed the plates at the feeding table in front of her chair, not in front of the children. Staff #1 sat down at the feeding table with the children and kept the plates close to her. She then began to individually feed the children (Children did not have access to their plates). As staff #1 individually fed the children, she skipped 3 children repeatedly, and the 3 children sat and watched her feed the other children. The children made no attempt to reach out for food or a plate and sat there very quietly and waited to be served. Even when they were skipped, the children did not react. The children simply waited for staff #1 to place the spoon in front of them and then opened their mouth. After spoon feeding the children for several rounds, staff #1 decided the children were done, stacked all the plates on top of each other, and threw them out. After this, staff #1 served 5 of the 7 children a cracker. *At 11:08 a.m., staff #1 roughly grabbed a child out of a chair at the feeding table and took the child to the diaper changing area. (This was the first diaper change the Surveyors' observed since 9:09 a.m.) *At 11:13 a.m., staff #1 abruptly grabbed a child from a chair at the feeding table, and put the child on the diaper changing surface. The staff member then walked to the other side of the room leaving the child unattended. At 11:15 a.m., staff #1 put the child back in the feeding table, rather than laying them down on a nap cot. *At 11:18 a.m., staff #1 grabbed a child from the feeding table, and took the child to the diaper changing area. After the diaper change, staff #1 walked the child to their nap cot and grabbed the child by one arm to lay them on their nap cot. *At 11:20 a.m., staff #1 walked over to a sleeping child who was laying on a cot and roughly flipped the child over onto their stomach. *Between 10:20 a.m. and 11:34 a.m., staff #1 walked out of the 1's classroom a total of 11 times, each time leaving all enrolled children alone in the room. On 09/27/21, staff stated the following regarding staff #1: *A child in staff #1's room always leaves the room shaking. *The children in staff #1's room are always crying. *A staff member tries to hug staff #1's kids, and staff #1 tells the staff member not to do that. *Staff #1 has slammed a child in the door and made it look like an accident. *The children seem afraid of staff #1. *Staff #1 runs her class like the military. *The children look depressed and are afraid to make any sudden moves. *The vibe is off in staff #1's room. *There were a "few" times children's faces were covered with blankets during nap time. *The children are quiet, and they seem off and do not move often. | |||

Findings: Per allegation and based on the Surveyors' observations and staff interview, staff #1 failed to supervise enrolled one-year-old children on 09/23/2021 when the following incidents occurred: *From 9:09 a.m. to 9:53 a.m., staff #1 sat on the area rug in the same spot, and did not move. During this time, staff #1's head kept dropping as if she was falling asleep. *From 9:09 a.m. to 11:35 a.m., staff #1 walked out of the room eleven times. *At 11:13 a.m., staff #1 grabbed a child from the feeding table, put the child on the diaper changing surface, and walked to the other side of the room leaving the child unattended on the diaper changing surface. *At 9:54 a.m., staff #1 walked to the door, and stood in the doorway and had her back turned towards the children while she was talking to another staff member. During this time, children were fighting and running around the room. Staff #1 stopped talking to the other staff member at 10:07 a.m. On 09/27/21, staff stated the following about staff #1: *I have seen staff #1 sitting with her eyes closed (while supervising children) during nap time. *Staff #1 walks out of the room often and leaves the children alone in the room. | |||
