Noncompliance Area: A renewal inspection occurred at the facility. A review of staff files was conducted. Staff #1 (previous 3/21/23, current 6/15/26) and Staff #2 (previous 4/1/24, current 6/16/26) did not participate in an updated health assessment at least every 24 months as required by regulation.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: An unannounced inspection occurred at the facility. A review of staff files was conducted. Staff #1 was observed to be caring for children unsupervised. Staff #1 does not have current pediatric first-aid/CPR training. This is evidenced by the previously documented pediatric first-aid/CPR training expiration date of April 2026.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed, staff #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff #1, staff #1 may not work in a child-care position at the facility.

Description: Age and Training

Noncompliance Area: An unannounced inspection occurred at the facility. A review of staff files was conducted. Staff #1 has not completed Pediatric first aid and CPR training on or before expiration of the most current certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring April 2026. Documentation of updated Pediatric first aid and CPR training is not on file.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Until such time as the required pre- service trainings are completed, staff #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff #1, staff #1 may not work in a child-care position at the facility.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, there was play equipment with cracks and sharp edges exposed seen on the larger outdoor play area.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, the agreements for 3 of the 5 child files that were reviewed were not signed by the operator.

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, Child #1 had an updated health report on file. However, the only health report on file Child #1 was dated 9/25/2019. Child #1 has attended the facility since 9/11/2018.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, the diaper changing pad that was being used for diapering in the infant/young toddler room was ripped with foam exposed; thus it could not be sanitized properly.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, Staff #1, Staff #2 and Staff #3 all had up to date health assessments, but their initial health assessments were conducted several months after they started working at the facility.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, peeling paint was seen in the preschool classroom.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Staff #1 was hired on 3/17/2018, but did not have an initial health assessment and tuberculosis screening by the Mantoux method for initial employment on file on 10/5/2018.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: Staff #2 was hired on 4/25/2018, but did not complete the mandated reporter training until 8/10/2018.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). All staff must complete the mandated reporter within 90 days of hire as required by the CPSL.

Noncompliance Area: A container of glass cleaner was seen in an unlocked cabinet underneath the sink in the cafeteria.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: In the outdoor playground area a tricycle with cracked plastic and two play tables with cracked plastic were seen. The gate for the toddler play area was also separated from the fence.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The facility fire drill log is currently up to date, but there was a lapse when a fire drill was held on 5/5/2018 and not again until 7/18/2018.

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.

Noncompliance Area: During a complaint inspection on 6/5/2018, staff had all of the children who were in care at the time outside in the parking lot area in front of the facility close to where cars enter and leave the parking lot. The infants and toddlers were in strollers on the walkway, but the preschool aged children were observed playing in the parking lot and the side of the building. There is not any fencing or other natural barriers to restrict the children from the unsafe areas where cars enter and leave the parking lot.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions.

Noncompliance Area: During a complaint follow-up inspection on the morning of 6/12/2018, Staff #1 was observed working in a classroom by herself with a mixed age group of 12 preschool and school age children.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times.

Noncompliance Area: During a complaint investigation on 6/5/2018, it was determined through staff interviews that the grass in the outdoor play area was knee high on staff four days earlier. This made length of grass too high for children to play safely in.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Two electrical outlets in the infant/young toddler room were observed to be missing receptacle covers.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: A small hole in the wall near the door was seen in the infant/young toddler room.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: During the renewal inspection on 10/19/2017, the hot water temperature measured 120 F in the lunchroom, 119 F in the baby room and 118 F in the boys bathroom. During a verification visit on 11/9/2017, the hot water temperature measured 117 F in the lunchroom, 115 F in the baby room and 118 F in the boys bathroom. During a verification visit on 12/5/2017, the hot water temperature measured 122 F in the lunchroom, 120 F in the baby room and 115 F in the boys bathroom.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: The play mats in the toddler 2 classroom and in the preschool classroom were observed to be ripped with foam exposed. The cribs in the baby room were not labeled for the use of each specific child.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child`s size, age and special needs. Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child`s parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Staff #1, Staff #2 and Staff #3 were all hired as assistant group supervisors in 2017. Each has a high school diploma. However, proof of child care experience prior to working at the facility and two years of experience working with children is not verified in the staff files.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person`s record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: The hot water temperature measured 120 F in the lunchroom, 119 F in the baby room and 118 F in the boys bathroom.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: The stalls in the boys and the girls bathrooms have rust on the bottom.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility`s outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: The facility fire drill log is current and up to date, but fire drills were previously not being held at least every 60 days. The fire drill log showed that a fire drill was held on 12/6/2016 and not again until 2/14/2017, then not again until 4/20/2017 and then not again until 6/28/2017.

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.
