A full inspection was not conducted. Please send a copy of the Statement of Services. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The fingerprint clearance card for 1 staff member was verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: New Rule Highlights, Statement of Services, Emergency Information and Immunization Record forms, Field trip requirements, Modification process, Notification to DHS in case of closure.

During the exit interview, the following items were discussed but are not limited to: General licensee responsibilities (director, director-designee, tuberculosis documentation, CPR-First aid training documentation, fire drills), Change processes, physical & administrative, and required timeframes, Main posting area documentation, Emergency Information and Immunization Record cards and attached Immunization records, Mandated reporting for suspected abuse or neglect, Availability of inspection reports, Staff files and training requirements, Staff-to-Child ratios, staff availability for supervision and administrative purposes, Drinking water availability, Hazards – maintenance, toxic/hazardous materials and equipment inaccessible, Diaper-changing area requirements, including postings, materials and layout, Appropriate developmental level materials available for enrolled children, Discipline and guidance requirements, Sleeping materials and equipment, Cleaning (facility, toys & equipment) and functioning plumbing, Accident and Emergency Procedure regulations, Illness log maintenance and use, Medications (permission forms, medication labeling, documentation), Transportation and Field trip requirements, General physical plant standards, Outdoor activity area (fencing, resilient surfacing, equipment, cleaning, hazards, shade), Fire and safety requirements, Facility areas and uses.
