Initial Comments: There were no deficiencies observed at the time of the initial monitoring inspection conducted on 5/8/2026. A full inspection was not conducted at this time. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: *Blue cards to have all required information. *Staff files and fingerprint clearance card documentation requirements. *Designated exits must be unobstructed at all times.

Initial Comments: There were no deficiencies observed at the time of the Initial Inspection conducted on 3/17/2026. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed but not limited to: 1. All required items for staff files 2. Broom/dustpan storage 3. Children’s sign-in and out records 4. Children's Emergency Information and Immunization records 5. Staff attendance records 6. Accessible drinking water 7. Monthly fire drills 8. Classroom rosters 9. Notification of pesticides 10.
