| Initial Comments: The purpose of this inspection was to conduct complaint #00182023 investigation on 8/26/2026. A focus inspection was conducted. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the facility director at the time of the inspection. The DES group size was evaluated at the time of the inspection. Ratios observed were: Infants - 1:5 1 - 1:4 2 - 1:5 3-5s - 1:5 There were 5 staff members interviewed during this investigation. There were 2 staff files reviewed during this investigation. There was 1 child’s file reviewed during this investigation. Others interviewed: The Compliance Officer contacted the complainant via email. Documentation observed was: *Diaper changing logs. *Requests for diaper changing supplies. *Facility messages via communication app. *Written staff statements. *Photos. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 2 of 2 allegations were unable to be substantiated due to a lack of sufficient evidence. There were no deficiencies found. This is subject to change pending programatic review. During the exit interview, the following items were discussed, but not limited to: *Diaper changing policies and procedures. *Staff communication best practices. *The use of incident reports to document. *The use of written permission for facility provided products. | |||
