Initial Comments: The purpose of the investigation was to conduct a complaint #00177932 investigation on 07-23-2026. There were no deficiencies found. This is subject to change pending programmatic review. A focus inspection was conducted. A copy of the Notice of Inspection Rights was provided during the inspection. The ratios observed were: *1's Room- 1:6 *2's Room- 1:6 *Gymnastics Room (3's)- 3:28 There were 5 staff members interviewed during this investigation. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 1 of 1 allegation was unable to be substantiated due to a lack of sufficient evidence. The following was discussed, but not limited to: 1. Importance of documentation.
