Findings: Based on the Surveyor's observations, the following hazards were accessible to enrolled children in the outdoor activity areas: * In the middle outdoor activity area on the south east corner, there was a white circular table with a 2" by 3" hole near the end of the table. * Large plastic pieces (2 ft by 4 ft) to a shed leaning against the chain link fence. * On the larger outdoor activity area, there was 6 ft. long black irrigation hose sticking out of the ground causing a tripping hazard.
