I have used this provider for more than 6 months

Staffing problems with frequent changes and being under staffed; both of which are not beneficial or good for the children

Rooms are out dated and have hazardous areas for children, making more prone for injury.

All walls and ceilings including doors and windows were not kept clean and in good repair. In space #4, the paint on the wall was peeling in four different places.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #4 and #5, one outlet in each space was not protected with a safety cover.

Outlets were observed uncovered in Space 3, 4 and in the stairwell leading to the preschool classrooms.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. I observed an Auvi-Q medication stored in a bookbag below 5 feet in Space 6 and an aerosol bug spray stored on top of a cabinet in space 5.

Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. While in the infant room I observed bottles labeled but not dated, one child's items had 1/7/23 listed.

All openings to the outer air were not protected against the entrance of flying insects. Space 4 was observed to have their back door propped open to their outdoor play space.

All equipment and furnishings were not in good repair. While in Space 2, which served infants, two large storage cabinets do not shut properly leaving space for children's fingers to get caught and/or pinched.

Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One child's Auvi-Q on site does not have the original container with the pharmacy label.
