All equipment and furnishings were not in good repair. I observed the pretend sink in the dramatic play area of Space 6 with a broken handle.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Outlets were observed uncovered in Space 3, 4 and in the stairwell leading to the preschool classrooms.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. I observed an Auvi-Q medication stored in a bookbag below 5 feet in Space 6 and an aerosol bug spray stored on top of a cabinet in space 5.

Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. While in the infant room I observed bottles labeled but not dated, one child's items had 1/7/23 listed.

All openings to the outer air were not protected against the entrance of flying insects. Space 4 was observed to have their back door propped open to their outdoor play space.

All equipment and furnishings were not in good repair. While in Space 2, which served infants, two large storage cabinets do not shut properly leaving space for children's fingers to get caught and/or pinched.

One staff was hired 8/29/22 and provided her TB screening on 9/15/22.

A copy of the safe sleep policy was not given to and/or explained to the parent of each child on or before the first day the infant attended the center. One infant's file did not have a signed safe sleep policy on file, the policy on file was not signed or dated.

The written feeding plan was not modified as the child's needs changed. One infant's feeding schedule was not current, depicting the child's current eating habits. The plan states breastmilk only, but the infant had food in a pouch to be given at a mealtime today.

Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The refrigerator thermometer in space 5, which stored that classroom children's lunch and drinks, was observed reading 50 degrees.

I have used this provider for more than 6 months

Staffing problems with frequent changes and being under staffed; both of which are not beneficial or good for the children

Rooms are out dated and have hazardous areas for children, making more prone for injury.
