The director walked in the room and told the staff in the room she would be in there to relieve her after covering a break next door, the staff in the room heard 'I'm in' and she walked out of the room thinking the director was present. The Director continued to walk through the restroom doors to the adjacent room, both not realizing what the other was doing, and left the 8 children in the room unattended. When a staff returned from their break about 15 minutes later, she observed the children and no staff present. She immediately notified the owner. | |||

| Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Upon my arrival I observed a purse sitting wide open on a chair in the lobby of the facility, in front of the front desk. Two medication prescription bottles were observed at the top of the opened purse. | |||
