A current activity plan was not posted for each group of children for reference. In space 4, where children ages 2 and 3 years old were located there was not an activity plan for the current week. In Space 10 where the School Age children were reportedly cared for there was an activity plan, but no date was listed just Week 20 - 24 which did not provide an accurate date.

When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. In Space 4 where children ages 2 1/2 to 3 years old were located there was only books in the Language center not allowing for a choice of different activities.

Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In Space 2 where Children who are 6 weeks to 23 months were located there was one bottle that had a name but no date on it.

On the play area where the school age children play there was some new mulch under two of the three swings. The mulch must meet the depth of at least 6 inches and did not; as well as the mulch was compacted therefore did not surfacing requirements.

In Space 2 where Children who are 6 weeks to 23 months were located there was a "plug-in" which is considered a substance that may be hazardous.

Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident on November 21, 2023, where a child was running and hit his face on the table did not have the following information included on the report. There was no witness to the incident listed. There was no location of the incident reported, nor was there a parent's signature.

Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. In the classroom serving two-year-old children there was mat that measured only 12 inches apart. During the observation the mats were moved to ensure that there was at least 18 inches apart.

All openings to the outer air were not protected against the entrance of flying insects. In the classroom serving children ages 6 weeks to 23 months old there was an open window with no screen on it. This was closed during the observation of the space once the rule was explained.

The depth of the loose surfacing was not based on critical height of the equipment. The mulch underneath a swing seat measured less than 1 inch in depth, rather than 6 inches as required by the critical height of the equipment on the playground where the toddler children were playing and, on the playground, where the school age children play there was only 2 inches of mulch under the swings rather than the required 6 inches.
