Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection the facility passed was on 3-7-22. A fire inspection was conducted on 3-22-23, however they did not pass inspection.

For children under three years of age, developmentally appropriate toys and activities were not provided as outlined in Rule. While in Space 2 I did not observe dramatic play material to use with the pretend kitchen sink, refrigerator, stove, I only observed babies and clothing in the interest area.

Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The refrigerator thermometer in Space 3 read 50 degrees, and do not believe it is working properly, the refrigerator itself seemed very cold.

All equipment and furnishings were not in good repair. Indoors, books were observed torn apart in several classroom. Outdoors I observed the swing set on the preschool playground is broken and swings are wrapped at the top of the swing stand. Hoola hoops were observed bent, buckets on the toddler playground were observed cracked. A pink heart push toy on the toddler playground was observed to have broken plastic wheels. A mat under the swings on the toddler playground was observed partially covered with dirt and mulch and the other end was sticking out causing a tripping hazard. The fencing around the A/C unit is wooden planks that are weathered, observed splintering, broken leaving sharp edges and the hinges have rusted.

A safe indoor and outdoor environment was not provided for the children. Outdoors I observed on the preschool playground tripping hazards, three wooden pieces are sticking out of the ground, it looks like they were wooden posts that have been sawed off. Tree roots by the toddler swing set were observed sticking out of the ground and are considered tripping hazards and protrusions. The outside of the building was observed with peeling paint on the lower trim of the building and on the doors, all inside the fenced in play areas. The chain link fencing is in poor repair in places, where it is unraveling at the base or not secure and the base is rolling up inwards, wooden pegs were used to tie the fencing down and they were observed broken leaving sharp edges and are sticking out and is considered a protrusion. Garden planks of the toddler playground were observed broken.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Three diaper creams in Space 2 were for children no longer enrolled in the program and have not been enrolled for more than 72 hours.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Today I followed a class, serving toddlers, out to the playground. The playground had not been checked, trash and broken equipment on the playground.

All vehicles used to transport children were not free of hazards. One of the buses used for transportation had a first aid kit that had a broken lid that did not close properly. The other bus had a cracked storage container that held the emergency safety panels to place by the bus during an emergency.

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. School agers are transported to and picked up from local elementary schools, many of the children's identifying information does not include photographs.

Enhanced staff/child ratios and group sizes were not met. Upon my arrival three classrooms were not meeting staff/child ratio. Two staff were in the front lobby speaking with the Director and went into classrooms when I walked into the lobby. Prior to them returning to a classroom Space 2, serving toddlers, had one staff with nine children, Space 1, serving two year olds, had one staff with fourteen children. I then walked into Space 8, serving four and five year olds, had one staff with sixteen children.

The depth of the loose surfacing was not based on critical height of the equipment. Mulch in the fall zones on all playgrounds measured less than 1 inch. The mulch has compacted so much it has turned into dirt, which is no longer considered resilient surfacing.
