About the Provider

Description: The purpose of Torah Day School of Phoenix is to create a warm, inviting, and loving atmosphere, where Jewish children from all backgrounds receive an excellent Jewish and General studies education, with an emphasis on Middos (moral character development), and within single-gender classrooms.

TDSP strives to create an educationally advanced Orthodox Jewish elementary school that offers single-gender classrooms starting from Kindergarten age.

The school will prepare students for the “bais medrash and the board room.” In other words, it will offer a superior Torah education and a superior secular education

The school will create an atmosphere that honors and respects students, teachers, and parents, and therefore fosters communal growth.

The school will be guided by halacha (Jewish law) and will seek to Mechanech (educate in a Torah fashion) its students to be strong, committed, observant Jews.

Initial Comments: The following deficiencies were observed at the time of the annual compliance inspection conducted on 08-26-2025, and are subject to changes pending programmatic review. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group size was compliant at the time of the inspection. The following was discussed, but not limited to: 1. Accessibility of equipment/tools, 2. Storage of outdoor maintenance tools (no children were present at the time), and 3. Changes to the rules (effective: 08-03-2025).

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 8/28/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1) The DCS portal is required to be used for all new staff and for any existing staff that never received DCS clearance results, 2) Discussed that the child's symptoms that manifest during an allergic reaction should be listed under the "reason" to administer the prescribed Epipens on the child's medication form, 3) Employment date, as related to obtaining fingerprint clearance card, verifying fingerprint card's validity, orientation completion, annual training cycle date and obtaining TB clearance, 5) The Empower program no longer subsidizes the fee, 6) Items stored in a bathroom must be stored inaccessible to children and in either a closed cabinet or on a shelf in a waterproof container with a lid, and 7) Procare sign in attendance- Parents whose fingerprints are not readable on the fingerprint scanner are required to sign in with a signature, per rule R9-5-306.B. The parent's can not sign in with only an assigned PIN code, as the Director stated those parents were doing. There were 6 staff files reviewed. 6 of the 6 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 08/29/2023, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: 1)Diaper Changing Sink water fluctuating, 2) The plastic windows on the blue tunnel on the Preschool playground, 3) Trash must be picked-up on the Basketball Court prior to the children using, 4) Water fountain on the Basketball Court, 5) Emergency Procedures, 6) Bleach used as an immediate sanitizer, 7) Personal label for sippy cups, 8) Broom bristles face up out of access to enrolled children., 9) All staff must document times of arrival and times of departure, 10)Staff must complete all questions on the Criminal History Affidavits.
