Findings: Based on Surveyor's inspection of the diapering areas, the following unrelated items were observed stored in the diapering areas: *Infant Room -A wash cloth and a bottle of body lotion on the diapering counter near the sink; *1-2 year old Room-A plastic pitcher, 2 sponges and a paint brush were stored on the diapering counter near the sink; and *2-3 year old Room- A plastic pitcher, a wicker basket, a plastic bin was found stored next to the diapering sink. A large quantity of more than 70 plastic toys including Legos, plastic toy dishes, toy letters and plastic animals were laying on a towel that was placed on top of the diaper changing table. The Director stated that the toys had just been cleaned and were placed on top of the diapering table to dry.

Findings: Based on Surveyor's inspection of the diaper changing areas, the Surveyor observed the following: *1-2 year old room- The soiled clothing container was not equipped with a liner and a tight fitting lid. The lid to the soiled diaper container was stuck open and the container contained soiled diapers; and *2-3 year old room- The diapering area was not equipped with a soiled clothing container. The staff acknowledged that the soiled clothing container was located in the hallway bathroom.

Findings: Based on Surveyor's observation and discussion with the Director and the infant room staff, they stated a 14 month old child who was is walking has been transitioning between the one year old room and the infant room each day. The Director stated that this child was currently in the one year old room but would return to the infant room later that day.

Findings: Based on Surveyor's observation, the soiled diaper containers in the Infant Room, the Toddler room and the 2& 3 year old room were not equipped with tight fitting lids. The lids were all stuck in the "open" position and the 3 containers had soiled diapers in them.
