Desert Christian Early Care and Education Center is located in Phoenix, Arizona on the campus of Desert Christian Fellowship Church, offering a Christian-based, non-denominational, Infant-Toddler Program that actively engages and encourages the faith lives of our families. Home and school form a partnerhsip for the love and care of each child. Our center is licensed by the Arizona Department of Health Services.

|Findings: Based on Surveyor's review of the architectural site plans for the 3 new outdoor activity areas, the Surveyor determined that the site plan did not include the following information: 1) The boundary dimensions of the property upon which the facility ' s physical plant is located; 2) The location of each driveway on the property; 3) The location and boundary dimensions of each parking lot on the property; 4) The location and perimeter dimensions of each outdoor activity area; and 5) The height of the southwest gate is incorrectly listed as 3.5 feet tall and is required to be at least 4 feet tall. NOTE: The actual gate is 4 feet tall but is incorrectly listed as 3.5 tall on drawings.|
|Findings: Based on a review of the modification request and on a discussion with the Director, the Surveyor determined that the licensee submitted a written request on 9/26/2018 to add a diapering area to room 2. The Director acknowledged that the diapering area had been used prior to the Department's approval of the area.|
|Findings: Based on the Surveyor's inspection of the proposed diapering area in room #2, the Surveyor observed the following: 1) A 5 foot long sheet of art paper and a wooden cut out were stored on the diaper changing counter; 2) The portable diapering table was equipped with a cloth buckle strap and with a mat that had thread stitched seams. Neither of the areas are waterproof and sanitizable; and 3) A one inch wide gap between the wall and the entire left side of the diapering sink counter.|

|Findings: The file for staff #4 lacked documentation of the staff's completion of the required DCS affidavit form.|
|Findings: Staff #4- date of employment-2/15/2018. The staff was issued a fingerprint card in 2017, prior to employment with this licensee. The Director acknowledged that Department of Public Safety had not been contacted within seven working days to verify that the fingerprint clearance card is valid.|
|Findings: The Surveyor determined the files for staff #1, #3 and #4 lacked documentation of the licensee's submission of the staff's Central Registry Background Clearance check and of the staff's Central Registry Background Clearance.|
|Findings: The Director informed the Surveyor that the outdoor activity area that the Department had approved was currently under construction. The Director indicated that the licensee had begun using 2 different outdoor areas (the basketball court and a fenced area near the parking lot as the outdoor activity area). The licensee failed to submit a written request for approval of these 2 outdoor areas to the Department at least 30 days before the intended change.|
|Findings: 1) The file for staff #3 (date of employment - 9/22/2017) contained documentation indicating that the staff's Mantoux skin test had been read on 9/25/2017 and it was determined that the staff did not submit documentation of the staff's evidence of freedom from infectious active tuberculosis on or before the staff's starting date of employment; and 2) The file for staff #4 (date of employment - 2/5/2018) contained documentation indicating that the staff's Mantoux skin test had been read on 3/23/2018 and it was determined that the staff did not submit documentation of the staff's evidence of freedom from infectious active tuberculosis on or before the staff's starting date of employment.|
|Findings: *2 & 3 year old room- The Surveyor observed 8 children present in the room at the time of the inspection but the roster had the names of 11 children listed as present; and *3 & 4 year old room - The Surveyor observed 14 children present in the room at the time of the inspection but the roster had the names of only 11 children listed as present.|
|Findings: The file for staff #2 (date of employment- 2/6/2012) contained documentation of training and indicated that the staff had completed only 8 of the required 18 hours of annual training for the period of 2/6/2017 through 2/6/2018.|
