| Findings: Ratio/Supervision. At time of investigation the facility was over child-staff ratio in one room. Proper supervision was not practiced as children used the restroom unaccompanied and out of direct staff sight. In addition, children were standing on chairs and toys that were not meant to be stood on. Safe sleep practice was not in use as children were sleeping next to doors and the door leading out of the toddler room's bell was not operational. Resolution: At time of follow up investigation, staff were actively observing the children on the playground and in the facility. A sensor had been replaced on the toddler door, & the facility had bells as back up for when/if sensor goes out. Children were not napping next to doors and the facility met child-staff ratio. | |||
