Findings: Child was seriously injured by biting. Parents were not notified. A staff member left the building, leaving children unattended. While the staff member was outside, one child bit another numerous times causing a serious injury. Staff did not call either child's parents, as is their policy. Staff did not notify Health and Welfare, as is ICCP rules. Staff did not obtain medical help for the serious injury. When the infant's parents picked the child up from the facility, they immediately brought the child to Urgent Care. Resolution: On 09/12/2018, a Health Inspector went to the facility to follow-up on the incident. The Owner and Manager were not present. While in the facility, the Health Inspector witnessed: -An infant crawling through the lunch room alone with no staff present. -A child crawling into the bathrooms with no staff members watching. -Children wearing only a diaper and nothing else. The Health Inspector noted that the room was chaotic and loud, and staff were yelling at the children; crying children were not provided comfort from staff. While the Health Inspector had a conversation with the Owner, the Health Inspector heard yelling, and looked up to see a staff member forcefully put a child on the child's sleeping mat and throw a blanket over them. Findings of the inspection include issues with inattentive personnel, inappropriate use of shouting to control children, rough handling of children, inability or willingness of staff to engage children, and unsanitary conditions at the facility. The owner of the facility plans to implement training for staff members, and follow the facility's policy of calling parents when a child is injured. The owner plans to complete a corrective action plan. STATE OF IDAHO DAYCARE LICENSE HAS BEEN REVOKED. FACILITY HAS BEEN TERMINATED FROM IDAHO CHILD CARE PROGRAM.
