| Findings: 20-00198/Supervision Concerns & Serious Injury. A child was not buckled into a high chair and was able to use another high chair to get out and fall. Staff members were in the room but failed to witness the incident. In addition, a volunteer staff member under 16 was not properly supervised by an adult as required. Lastly, children were left unsupervised on multiple occasions. Resolution: Provider will be utilizing high chairs that are low to the ground to prevent this from happening again and also make sure all children are buckled in. Provider was advised that underaged staff must be supervised by an adult who is physically in the room. As well as children must be adequately supervised at all times and staff may not leave the room even briefly. | |||
