A safe indoor and outdoor environment was not provided for the children. I observed a long, pink rubber string that was tied to the climbing structure on the playground.

Incident logs were not completed and maintained as required. I observed one incident report that was not recorded on an incident log. C. Williams also stated that there was a second incident that they did not have a copy of the report to put on the log due to it being in the office.

The only documented playground inspection was in August 2023. There was not a playground inspection recorded for September 2023, October 2023, November 2023, December 2023, January 2024, or February 2024.

Menus for all meals and snacks were not current or posted where easily seen by parents and cook. I did not observe a posted menu.

Equipment and furnishings were not sturdy, stable and free of hazards. On the large play structure, I observed several spots that had exposed rust. I observed peeling plastic on the steps which was also exposing rust.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. I observed one employee who's first aid certification expired 08/2023.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. I observed one substitute file that did not have a signed acknowledgment of reviewing the Shaken Baby Syndrome Policy prior to caring for children.

Fire drills were not practiced monthly and/or the drill record was incomplete. I observed that fire drills that were conducted monthly were not recorded on the fire drill log from November through March.
