Oak Summit Learning Academy specializes in fostering development, building self-esteem, encouraging imagination, and providing loving attention and safety. All equipment and furnishings were not in good repair. On the playground, I observed peeling paint and rust on the yellow truck climbing equipment. I observed the gate leading to the playground contained an opening measuring 4 1/2 inches wide between the gate and post.

New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. I did not observe documentation of orientation in the file of one staff member who started 10/4/22.

Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. In three children's records, I observed the emergency medical care information was not updated annually.

Enhanced staff/child ratios and group sizes were not met. In room 2, a teacher was not present in the classroom where I observed 4 children ages one to two years of age were sleeping. During the visit, the assigned teacher returned to the classroom.
