| Findings: There is one child born after 1/1/2015 with no documentation on site of a lead test having been done. It was determined that the doctors office gave the parent a referral for a lead test in 2017 and the doctor's office has no record of having the results of the test. The provider contacted the parent who is now planning to obtain a new referral to have the testing completed. Please forward, within 30 days, written corrective action that the blood lead testing was completed for the child in question. The provider contacted the doctors office and the parent while Lic. Spec. was on site and both conversations were conducted on speaker from the provider's telephone, and Lic. Spec. heard the conversations. | |||

| Findings: The provider added a wooden climber and swing set which is in the back yard. The wooden, above the ground, platform has railings. There are entrapment areas between some of the wooden slats on the railing. There are to be no openings greater than 3.5 inches. There is at least one opening that is about 5" wide. Discussed that any opening between 3.5" and 9" could be an entrapment area. Provider plans to add screen on the inside of the railing to make the entrapment area inaccessible to children. Please forward written corrective action within 30 days. The platform is about 5' off of the ground. There is an above ground pool in the back yard and it is enclosed with fencing around 3 sides and the house is the 4th side. The gate into the pool area is self-closing and self-latching. The gate was tested and worked properly today. There was a large planter on the ground that was against the stair railing leading to the pool and it may have allowed a child to climb into the pool area. Provider immediately relocated the planter. Please keep the area free of any possible climbing items. The hot tub has an approved locked cover on it. Please keep the outdoor area safeguarded at all times. | |||
