Description: Several cots in the 4 year old room do not have sheets present while children are resting. There are several waffles present in a bowl in the refrigerator without a covering. There is a slide and a climber present on the playground without impact absorbing material. An electrical wire is hanging from the building in the outdoor play area with part of it laying directly on the grass. The ends of the wire and still attached to the building with a portion disconnected and hanging on the ground. The swings removed from the swing set in the outdoor play area are accessible on the ground next to the swing set and could pose a strangulation hazard. |
