Description: At our Center Township facility we serve children from ages six weeks through those entering 6th grade. We have based our childcare on unique, child centered lessons and experiences. The parents will get me an updated copy of Child #1's immunizations. In the future, I will make sure that all parents are aware that we need documentation from the doctor's when they get shots of any kind. We are requesting that parents take an updated physical form when they get any immunizations. Child #2 has an annual physical scheduled at the end of January. If child had a flu shot in 2019, this will be on the next shot record. If not, parent will sign a letter of exemption. In the future, all child files will either have a letter of exemption or a current flu shot on file.

In the future we will submit a work order to the municipal authority, and notify our certification rep whenever repairs have been made.

When asked what time the school bus arrived, Staff Member #1 informed the Certification Representative 8:25. The Certification Representative asked that Staff Member #1 take the 8 children back to their room until it was time to leave for the bus. The 6 younger school age children were in the room that measured for 5, and the 8 older school age children were in a room that measured for 7.

The two children who were over capacity were moved on the day of the inspection to another room where they would be in compliance for capacity, supervision and ratio. In the future, we will make sure that all rooms stay in capacity per the amount of children allowed in the rooms. We will also make all new staff aware of the room capacities and how to stay in capacity throughout the day.

The violation was already corrected at the time of inspection with a updated physical being on file. The child is now two and is required to get a physical every 12 months. In the future, I will make sure that all enrolled children will get their physicals in a timely manner. Younger toddlers and infants will get a physical and have it turned in at least every 6 months.
