Findings: Upon inspection of the outdoor area, Surveyor 1 noted the following: -five 50 lb bags of lime. All bags were labeled "Danger" for causing skin irritation, respiratory irritation and possible eye damage; -broken glass spread over the ground of one area of the activity area. The Provider reported that an adult resident of the home had broken a window causing glass to shatter. -uninstalled blinds with cords.
