|Findings: Upon review of the group home's Emergency, Information, and Immunization Records, Surveyor 2 noted that the cards of Child 1 and Child 2 did not list their dates of enrollment nor the name of the individual to contact in case of injury or sudden illness. *Previously cited on 12/5/2018 and 12/5/2017.|
|Findings: Upon inspection of the outdoor area, Surveyor 1 noted the following: -five 50 lb bags of lime. All bags were labeled "Danger" for causing skin irritation, respiratory irritation and possible eye damage; -broken glass spread over the ground of one area of the activity area. The Provider reported that an adult resident of the home had broken a window causing glass to shatter. -uninstalled blinds with cords.|
|Findings: During the inspection of the outdoor activity area, the Surveyors observed a push and ride toy and a tricycle that had broken and jagged parts.|
|Findings: During the inspection, Surveyor 1 requested the group home's written instructions for feeding an enrolled infant that was present at the facility at the time of the inspection. The Provider reported that they had not requested written instructions from the child's guardian.|
|Findings: During the inspection of the outdoor activity area, Surveyor 1 measured a vertical gap greater than 4 inches in a gate leading to a section of the home's outdoor space that is not licensed and has heavy tools and construction material present.|
|Findings: During the inspection of licensed spaces in the group home, Surveyor 1 noted that there was no smoke detector in the living room nor in the bedroom where a child was napping when the Surveyors arrived at the group home. *Previously cited on 12/5/2018.|
|Findings: During the inspection of licensed activity areas in the group home, Surveyor 1 noted that an unused outlet in the dining area and another in the bedroom where a child was napping were not covered with safety plugs. *Previously cited on 12/1/2016.|
|Findings: During the inspection of the group home's bathroom, Surveyor 1 noted that there was a can of hairspray that was labeled as flammable accessible to children.|
|Findings: During the inspection of the group home's bathroom, Surveyor 1 noted that there were no paper towels present in the bathroom.|
|Findings: The Surveyors observed that the diaper changing surface was not within 15 feet of running water and dispensed soap.|
|Findings: Based on Surveyor 1's interview with the Provider, it was determined that a toy chest was being used as a diaper changing surface. Both Surveyors observed that the surface was not sanitizable due to the grooves and crevices present on the surface of the toy chest.|
|Findings: Upon reviewing the group home's diaper changing log, Surveyor 1 noted that the last entry in the log was dated 11/8/2019. The Provider reported that there was no documentation of diaper changes from 11/11/19 through the day of the inspection on 11/18/19. *Previously cited on 12/1/2016.|
|Findings: During the Mid-Year inspection, the Team Leader observed that the crib in which a child was resting, had a crib mattress with a sheet that was loose and not tight fitting.|
