Findings: Upon inspection of the outdoor area, Surveyor 1 noted the following: -five 50 lb bags of lime. All bags were labeled "Danger" for causing skin irritation, respiratory irritation and possible eye damage; -broken glass spread over the ground of one area of the activity area. The Provider reported that an adult resident of the home had broken a window causing glass to shatter. -uninstalled blinds with cords.

Findings: Based on Surveyor 1's interview with the Provider, it was determined that a toy chest was being used as a diaper changing surface. Both Surveyors observed that the surface was not sanitizable due to the grooves and crevices present on the surface of the toy chest.

Findings: Upon reviewing the group home's diaper changing log, Surveyor 1 noted that the last entry in the log was dated 11/8/2019. The Provider reported that there was no documentation of diaper changes from 11/11/19 through the day of the inspection on 11/18/19. *Previously cited on 12/1/2016.
