|Findings: Surveyor 1 observed an child sleeping in a crib with a bottle containing milk hanging out of the child's mouth. The Provider acknowledged that she did not have written permission to allow milk in the bottle during nap time.|
|Findings: Surveyor 2 reviewed two personnel files and observed the following: * The Provider had 6 hours of documented training between 02/2017 - 02/2018 (start date 02/19/2009). * Staff 1 had 11 hours of documented training between 02/2017 - 02/2018 (start date 02/19/2009).|
|Findings: Surveyor 2 reviewed the children's emergency cards and observed the following: * Child 1 was missing a date of enrollment. * Child 2 was missing the name of the first person to be contacted in case of injury or sudden illness of the child. * Child 3 was missing the name of the first person to be contacted in case of injury or sudden illness of the child.|
|Findings: The Surveyor reviewed three personnel files and observed that all three staff members (Provider, Staff 1 and Staff 2) were missing documentation of the required submittal to the DCS Central Registry for a background check.|
|Findings: The Provider acknowledged that herself and Staff 1 have not maintained their attendance records. The Surveyor observed that the last date they signed-in was 01/01/18.|
|Findings: The Surveyor reviewed three personnel files and observed the following: * The Provider had three hours of documented training between 02/2016 - 02/2017 (start date 02/19/09).|
|Findings: The Surveyor inspected the first-aid kit and observed it was missing sterile gauze rolls and antiseptic solution or sealed antiseptic wipes.|
|Findings: Based on a conversation with the Provider, the Surveyor was told that she was not maintaining a diaper log.|
