| Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. In space #1, a small electric fan was observed on top of the water dispenser beside the handwashing sink. The electric fan was turned on during the visit and was not mounted nor had a mesh guard. The Lead Teacher removed the electric fan during the visit. | |||

| Children were not adequately supervised at all times. One (1) child four (4) years of age returned to the classroom, space #1, alone and unsupervised after lunch while the Lead Teacher, Teacher and remaining fourteen (14) children were visiting the restrooms adjacent to the gymnasium. The Child Care Consultants accompanied the child to the restrooms to join the teachers and children. The teachers were unaware that the child had left the group. | |||
