Equipment and furnishings were not sturdy, stable and free of hazards. Rust was present on the fence facing the parking lot. Protrusions were observed on the bottom of the gate leading to the parking lot.

Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. A five (5) inch gap was observed in the gate leading to the parking lot. A four (4) inch gap was observed in the gate leading to the garden area behind the building.

The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. One (1) wooden table was overturned and rotted on the playground. Several pieces of trash were observed near the play structure and swings.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In classroom space 4, one (1) power strip and one (1) outlet were not covered. In the hallway, two (2) outlets were uncovered. In classroom space 5, three (3) outlets were uncovered.

The depth of the loose surfacing was not based on critical height of the equipment. The slide had two (2) inches of mulch, mixed with pine needles. All four (4) swings had one (1) inch of mulch.

When I remember Hillcrest headstart I remember my childhood. Here I cried like a baby when my parents dropped me off every morning, but those tears were all worth it. At this head start I learned my first words in English, I made my first friends ever, I lost my fear of being around people other than my family. I got very well treated. Now I'm a high school graduate, future College student. Words stay short to how thankful I am with this head star t because here is where my education began.
