Initial Comments: The following deficiency was observed at the Compliance Inspection conducted on 12/17/2025, and is subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The following was discussed but not limited to: * The DCS portal that was effective 7/2024 is required to be used for all current staff. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.

Initial Comments: The following deficiency was found at the time of the Compliance Inspection conducted on 1/8/2025, and is subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation of correction is required to be submitted through the Licensing Portal within 10 days of receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1)The fingerprint clearance cards for staff #2 and #3 will expire in February 2025 and are required to be renewed prior to their expiration date, and 2) The Anniversary fee is due in Feb 2025. There were 3 staff files reviewed. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Program self assessment link was emailed to the Provider during the inspection.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 1/31/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 3 of 3 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection.
