We wanted to recognize that the employee did have a valid evaluation from a "Physical for Sports". Our DHS Health Form was obtained by employee and signed by a doctor. The new health form/assessment was given to the Director and now includes an examination for communicable diseases and the results of that examination and the physician's or CRNP's assessment of the person's suitability to provide child care.. |
