The doors to the storage shed were off and laying next to the shed accessible to enrolled children. On the Preschool outside activity area, large metal doors with sharp metal edges were being stored between the white fencing and the storage unit facing west accessible to enrolled children.|
|Findings: Based on facility documentation and the Surveyors' observations, it was determined that the roster in the 1's classroom did not reflect the number of children present in the room (5 children present, 4 children listed on the roster).|
|Findings: Per allegation and based on facility documentation and staff interviews, it was determined that in the one-year old's classroom, a enrolled child had been bitten by another enrolled child on 08/8/19 and 2 enrolled children had been bitten during the morning of 8/22/19.|
