Observed standing water at the foot of the stairs along the wall by the stove, in front of the counter, down the hallway that leads to outside, on the other side of the counter by book shelf and booth seating. There was also traces of water observed behind the counter in the cooking areas.

Observed Staff primarily responsible for food preparation has not completed food handler training. Staff reported they are currently in the middle of stating food handler training.

Plan: Person in charge is going to clean black substance off the wall and will make sure to check the walls and clean as needed. Corrected at visit. Tortilla chips were picked up at the time visit. Staff will vacuum daily and as needed.

Description: 340:110-3-304(a)(1) - The facility and vehicles are clean, sanitary, and free of clutter and litter, including paper accumulation. The facility and equipment do not have accumulations of dust, dirt, soils, food particles, and other debris.

Observed a black substance on the wall and on the bug bulletin board trim that is also hanging on the wall, above the hot water tank. Observed crushed up tortilla chips on the floor upstairs.
