Plan: The milk was thrown away immediately and is now being checked prior to service. Training was provided by Sandy Bonham, INCA Area Supervisor who was on-site on October 23rd. INCA CACFP Manager provided training specific to dates of expiration on the milk and how to dispose of it. She also addressed the issue of checking the carton to be sure the in-date milk is good also. This was completed on October 23,2020 by Crystal Cortes. Follow up has been done by Crystal to ensure that the correct process is followed. 12/19/2020

observed 20 or more piece of pink insulation strung through out the outdoor play. Children were observed to be outdoor playing upon arrival. Staff reported birds are somehow getting around the roof and pulling the insulation out.

Only teaching personnel count in ratios, unless the requirements specifically state otherwise. The program is required to maintain ratios at all times.

observed one staff on the playground with 14 children. Ratio observed 14:1 with thirteen 3-year-olds. Second staff who was suppose to meet ratio was observed in the classroom doing lap-d assessment with a child. Second staff reported they were finishing a task and told the other staff to go ahead and go outside.

In the future teachers will double check to make sure all forms are completed and dated annually.

Director will contact manager to see if they can assign staff to the program or if staff will needs to assign themselves to the program. Once assigned to the program Director or manager who is over seeing the registry will need to verify employment. In the future director will speak with manager over the registry to see if there is a system for adding and verify employment on the registry.
