Description: Evergreen Elementary consists of over 600 energetic Pre K-5th grade students. Our goal is to develop life-long learners who will lead environmentally conscious lives. At Evergreen we strive to provide a rigorous academic program and foster environmental stewardship. Our building is truly a â€œteaching toolâ€ whose green components are visible and integrated into the curriculum. Evergreen was awarded LEED â€œgoldâ€ status in October 2010 which is the leading-edge system for certifying the greenest performing buildings in the world. |Findings: All emergency cards were reviewed. 3 cards need an update date next to the parent's initials. 7 children need their physician's telephone number on their emergency cards. One child needs an update on his emergency card.|
|Findings: 2 aides did not have a staff orientation in their files.|
|Findings: An aide needed 3 more hours of continued training by 3/31/19. He hasn't completed it yet. He needs to complete this 3 hours ASAP.|
|Findings: 28 children's records were reviewed. One child needs the parent part of the health inventory completed.|
|Findings: Center was prepared to just have cheese sticks and water for snack. Specialist reminded center that 2 food groups need to be represented at a snack. The center then offered juice with the cheese sticks.|
|Findings: There is a bottle of Benedryl that has no child's name on it and director (new to the site) didn't know who it belongs to. Center needs to be sure that any medicine that comes into the center is labelled with the child's name.|
|Findings: A child's Epi Pen expired in 10/2018. This needs to be returned to the parent and a new one received if necessary.|
|Findings: First aid kit needs a large, thick gauze pad (like a sanitary napkin).|
|Findings: The freezer didn't have a thermometer.|
|Findings: There is Benedryl for a child but no medication authorization form for it. There was an Epi Pen for a female child but no medication authorization form for it.|
