Findings: There are two staff members, K. W. and S.M. that it was stated no longer work at the center and the program has not provided documentation of them ending employment within the 5 working days. The program must submit a change form removing these staff members immediately

Findings: While the licensing specialist was present the staff member in the infant room left the room with 5 infants alone to go fill up bottles with water leaving them unattended for 1 minute. It was brought to staff's attention that they cannot leave the children unattended and this was corrected on site when the staff member returned and was talked to by another staff member.

Findings: When the licensing specialist arrived on site, the director was not present so it was asked who was in charge in her place. Two staff members (G.B and T.G.) stated it was either one of them. G.B. became the one designated. While on the phone with the director during the course of the inspection it was explained to the licensing specialist that T.G. was the staff member in charge, who is a lead teacher. It was explained that a written statement identifying this must be submitted to the licensing specialist and this must be reviewed with staff so that they are aware of their roles and responsibilities while the director is not present.
