|Findings: LS observed 2 ceiling tiles needing to be replaced in the hallway where children enter the center, located over the director desk. LS smelled an odor when entering the building front doors. Throughout the building there is a strong odor. LS went downstairs to where the infants are located and there was an odor of dampness and possible mildew. LS questioned the center director in regards to the odor downstairs; the center Director stated that downstairs has not flooded.|
|Findings: Observed changing pad with rips, needs to be replaced.|
|Findings: A written diapering procedure was not posted.|
|Findings: Update staffing pattern in infant room 5a for correct staff.|
|Findings: Observed 1 light out in room 1, broken floor board in room 1 and room 2 that needs to be replaced, carpet in infant room 5b is torn and chipping paint under window in infant room 5B. Within 5 days a letter of corrections of how you will fix the 3 issues.|
|Findings: Director missing 3 hours of training.|
|Findings: 1 staff person needs 3 more continued training hours.|
|Findings: Observed 7 children under the age of 18 months in a group. Corrected during inspection. Over capacity for group size for 2 year olds in room 4.|
|Findings: Emergency numbers were not posted near the phone in infant room 5a.|
|Findings: Observed holes in the changing pad in infant room 5A needs to be replaced, also observed 3 cribs needing fitted sheets.|
