We self-reported the incident immediately to FSSA. We contacted Child Protective Services and made a self-report. We instituted stricter than required bottle procedures. Staff involved were immediately removed and are no longer employed with Easterseals Rehabilitation Center. All staff in infant classrooms have been retrained and have signed a statement of understanding bottle preparation and correct feeding procedures.
