The file for child #3, a 23 month old, had an immunization record that lacked documentation of the 4th dose of the DTAP vaccination by age 18 months and the 1st dose of the MMR vaccination by age 15 months in accordance with the schedule recommended by the ACIP.

Noncompliance Area: After diapering, staff person #2 did not have an older toddler wash their hands.

Noncompliance Area: During lunch time, it was observed that staff person #2 was preparing lunch on the same surface as the changing table. Staff person #2 stated that the surface was sanitized between uses and the changing pad was moved to prepare food on the counter top.

Correction Required: The diapering area may not be used for food preparation or food service.

Noncompliance Area: On 12-13-19, it was observed that the operator did not have a fire extinguisher located in the kitchen. The fire extinguisher was located on a high shelf in the child care space downstairs. This condition at the facility poses a threat to the health or safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.
