| Initial Comments: The following deficiencies were observed at the time of the Initial inspection conducted on May 20, 2026, subject to changes pending programmatic review. Please complete the Plan of Corrections on the Licensing portal within 10 days of receiving this Statement of Deficiencies. 2 of 2 fingerprint clearance cards reviewed were valid through the DPS website at the time of the inspection. The license will be issued upon receipt of verification of all corrections, including photos and documentation. The following was discussed, but not limited to: *Stock medications, *Padding under indoor slides and climbing equipment, *Vehicle insurance must include commercial usage, *Diapering areas must be within 15 feet of a sink. | |||
