Ensure the diaper changing area is cleaned with soap and water and disinfected after each diaper change. Corrective Action: Ensure a complete Medication Administration Record (MAR) is obtained for all medication. Four children need complete MAR's on file. Ensure medication is labeled with children's names and stored out of children's reach. No diaper cream was labeled. A can of Aquaphor was observed on the teacher desk in the two-year-old classroom and was accessible to children. This was corrected when the Aquaphor was moved. A can of corn starch was observed on the changing table's shelf. The corn starch was accessible to children. When asked, staff stated it is used on children when they have a diaper rash. No written parent permission was observed on file for the corn starch. This was discussed and corrected when the corn starch was removed.
