Findings: Based on facility documentation and the Surveyor's observation, after a review of the Statement of Child Care Services, it was determine the following was lacking: -A description of the liability insurance required by R9-5-308 that is carried by the licensee and a statement that documentation of the liability insurance coverage is available for review on the facility premises. -A notice stating inspection reports are available on-site. -A provision stating that the facility is regulated by the Arizona Department of Health Services including the Department's local street address, city, state, zip code, and local telephone number. -The procedures for notifying a parent at least 48 hours before a pesticide is applied on a facility's premises.

Findings: Based on the Surveyor's observation, the following rooms were not maintained free from hazards: -In Room's 615 and 617, there was 1 ceiling tile warping and curl, causing the tile to not be secured to the ceiling. -In Room 621, there were 2 ceiling tiles not secured to the ceiling. -In Room 625, there was 1 ceiling tile not secured to the ceiling. -In Room 629, there were 2 ceiling tiles not secured to the ceiling. -In Room 630, there were 6 ceiling tiles not secured to the ceiling and 2 ceiling tiles warping and curl causing the tile to not be secured to the ceiling.

Findings: Based on the Surveyor's observation, on the Toddler playground lacked the required resilient surfacing (woodchips): -The pink slide lacked 3" of the required 6" of resilient surface within the fall zones. -The green slide and green ladder 4" of the required 6" of resilient surface within the fall zones. -The blue slide 2" of the required 6" of resilient surface within the fall zones. -There was a small blue slide on the green grass 6" of the required 6" of resilient surface within the fall zones.

I have used this provider for more than 6 months

The morning staff at the preschool is pretty good just don’t have your child stay for “aftercare” they hire young inexperienced kids. Multiple times when picking up four aftercare staff will be standing in the doorways of classrooms talking to each other while the kids inside are doing whatever they want! Administration is oblivious to this…Summer camp for elementary kids is a joke because it is run by the same young kids who do nothing with the kids but cram them all in one small room and talk to each other. You are overpaying for a glorified daycare with a warm body.
