The Empower Self-Evaluation survey was emailed to the licensee at the time of the inspection. The Notice Of Inspection Rights were provided to the licensee at the time of the inspection. Initial Comments: There were no deficiencies observed during the Complaint Investigation for Case Number 00086577 that was conducted on 07/31/2024 and 08/01/2024 and are subject to changes pending programmatic review. A complete inspection of the facility was not conducted. The following staff-to-children ratios were observed: EHS Infants: 1:3 Infant 1: 2:6 Infant 2: 1:4 EHS Toddler 1: 2:6 EHS Toddler 2: 2:7 Toddler 1: 2:8 Toddler 2: 2:13 Twos 1: 2:13 Twos 2: 2:16 Preschool 1: 1:10 Preschool 2: 1:10 Pre-K 1: 1:11 Pre-K 2: 1:13 School-age: 3:31 The Department contacted the complainant on 07/01/2024. The Director and District Manager were interviewed. Documentation reviewed: 3 staff files 2 children's files Accident/Injury and Illness Log for 2024 Diaper changing logs for 06/05/2024 Watch Me Grow App communications from 06/05/2024 Incident reports for 06/11/2024, 04/03/2024, 03/21/2024, 03/13/2024, Text messages from 06/11/2024 The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. Upon completion of the Complaint Investigation for Case Number 00086577, it was determined from the Director and District Manager interviews, facility documentation, and the Compliance Officer's observations that 3 of 3 allegations lacked sufficient evidence to be substantiated.
